Intellicure CMO, Caroline Fife, MD, Talks Health Care Reform at Annual Fall Symposium on Advanced Wound Care

Caroline Fife, MD, will be headlining two continuing education sessions at the upcoming Symposium on Advanced Wound Care in Las Vegas October 16-18, 2014. One session sure to draw a crowd focuses on recent health care reform changes and how they will affect wound care physicians and centers. Topics will include mandated initiatives such as wound care quality measures, sunshine physician payment rules, and new policies regarding reporting to be implemented by CMS in the coming months. Joining Dr. Fife for this informative session on October 17 at 9am PDT will be Guy Clifton, MD, Houston neurosurgeon, RW Johnson Health Policy Fellow, and author of Flatlined: Resuscitating American Medicine.

"Today we are in the middle of a fundamental restructuring of outpatient payment away from volume based payment to one based on value," stated Caroline Fife, MD. Dr. Fife will also be speaking on “Lymphedema Therapy: What You Know, What is New?” Dr. Fife and Dr. Eva Sevick have worked to develop technology that allows real time imaging of the lymphatics using infrared light. This technology has been used to identify more effective treatments for lymphedema, discover new genes that cause primary lymphedema, and provide insights on the way that cancer might spread. The session on October 18 at 10:10am PDT will address the complex physiology of the lymphedema patient and available treatment strategies.

Bridging the "Quality Gap" in Diabetic Foot Ulcer Care

There are 1.3 million new cases (incident cases) of diabetes that are diagnosed annually in the US. About a quarter of these patients will get a diabetic foot ulcer (DFU) at some time in their life, and about 15% of DFUs will result in amputation. Sadly, amputation is not the worst part of the story. Nearly half of all unhealed DFUs result in death within 5 years. In fact, a DFU is more likely to kill than most cancers. Diabetes has become an epidemic in the US so it’s no wonder that there is an increasing number of advanced therapeutics available for them. However, none of these interventions are likely to be effective in the absence of appropriate, basic quality care.

The first two interventions listed for DFUs by The Wound Healing Society (WHS) Guidelines (both with Level 1 evidence) are: 1) rule out arterial disease and 2) employ an acceptable method of off-loading (Wound Rep Reg (2006) 14:680–692). In decreasing order of effectiveness, off-loading options include total contact casting (TCC), custom removable orthotic walkers (CROW), insoles, half shoes, crutches/walkers, or post-operative shoes. I participated in a consensus panel, spearheaded by Rob Snyder, DPM to develop practical off-loading recommendations. The manuscript produced will be published in November. The information was summarized in a special session at the SAWC and the recording of that session is now available for CME credit.

Part of our goal was to determine whether there really was a “gap in practice” that needed to be bridged by a clinical quality measure. In 2009, Intellicure proposed that CMS include  DFU off-loading  in the Physician Quality Reporting System (PQRS). CMS rejected this as a quality measure because it had not been endorsed by a national quality organization. In addition, one of the things CMS wanted to see was whether there really was a "gap in practice". It isn’t necessary to create a quality measure to encourage a behavior if everyone already DOES that intervention. The results of 9 total contact cast studies show that the average healing time for DFUs is 43 days and that 88.9% of ulcers healed with TCC. These results outstrip every advanced therapeutic on the market. Given the overwhelming data favoring the simple and inexpensive practice of off-loading in the management of DFUs, it didn’t seem possible that there was a problem getting doctors to implement it. So, we looked at U.S. Wound Registry (USWR) data contributed by Intellicure clients.

 

We evaluated all patients with DFUs from 2009 -2011 (2 years), looking at whether off-loading was documented and if so what was used, infection rates, and wound outcomes. A total of 11,784 patients with 25,114 DFUs were analyzed (~2 wounds per patient). When total contact casting was used, the majority of patients undergoing TCC (58%) had Wagner 1 lesions with Wagner 2 and 3 lesions combined representing another 41% of all patients undergoing TCC. However, TCC was only documented in 16% of all DFUs. The most commonly documented method of off-loading was a post-operative shoe (36.8% of DFUs were treated this way). Only 61% of hospital based outpatient wound centers used TCC at all. This is particularly unfortunate since DFUs that were NOT treated with TCC had an amputation rate of 5.2% and DFUs that were treated with TCC had an amputation rate of only 2.2% (p = 0.001). Non-TCC treated wounds were much more likely to have wound infections (2.6 episodes per year) compared to TCC treated wounds (1.6 episodes). We estimate that only 3.7% of DFUs which were eligible for TCC received it.

What we learned from the USWR data is that TCC is vastly underutilized in DFU treatment and there is a BIG “gap in practice”. Data suggested that the easier to apply kits (like the TCC-EZ) may increase the percentage of DFUs treated with TCC. We also learned that in a real world setting, TCC decreases amputation rate and infection incidence. While we did not perform a detailed cost analysis, it is safe to say that the above effects of TCC translate to total cost savings. We are going to need cost savings if we are going to save Medicare for ourselves as we grow older.

As part of health care reform, CMS will shift from “paying for volume” to “paying for value.” How can we bridge the gap in off-loading practice? We created a DFU off-loading quality measure through the USWR Qualified Clinical Data Registry (QCDR). Physicians can report DFU off-loading as one of the 9 quality measures which will allow them to avoid a 2% Medicare pay cut in 2016.

The good news for Intellicure clients is that while doctors using other EHRs have to convince their vendor to program quality measures into their EHR, Intellicure has already done it for you. We have been piloting quality measures for years as part of our “Do The Right Thing™” initiative.

John F. Moore has joined Intellicure as the Chief Administrative Officer. John brings a wealth of knowledge to our company, and we are very excited to have him on our team.

Previously, John was the principal consultant for mergers, acquisitions and strategic alliances at Catalyst Consulting. He joined Catalyst in February 2013, bringing his many years of practice and financial management experience to the highly regarded consulting group in Austin, Texas.

Prior to joining Catalyst, he held CEO positions for the largest cardiothoracic and vascular surgery group in Austin as well as the largest OB/GYN group in Austin. He led a very talented administrative team to merge the five largest OB/GYN groups in 2010, creating a very successful $42 million group of 40 physicians with seven locations and 340+ employees.

 

John is the past president of the Texas chapter of Medical Group Management Association (MGMA), a professional organization of medical practice executives representing nearly 500 members who manage the offices for over 3,000 physicians in Texas. Patient Portal- Get A Jump Start On Meaningful Use Requirements

How would you like an easy way to start the New Year off on a good foot, especially when it comes to Meaningful Use?

One of the Meaningful Use Stage 1 requirements is to provide patients with an electronic copy of their health information upon request. Patient Portal is Intellicure’s answer to this requirement.

To give a patient access to our Patient Portal, all you need to do is click Reports in the upper left-hand corner of either their initial or follow up encounter and then choose Patient Portal Access. You will then be asked for the patient’s email address if they have one or you can choose “Cancel”. This will pull up Patient Portal Instructions with a web address the patient can go to in order to see their health records online. When your patient accesses their records online, they will be able to see their Continuity of Care Document (CCD). Included in the CCD is information about patient’s problems, their family history, allergies, medications, vital signs, and diagnostic test results.

The Importance of Level 2 Reporting

This month I want to say thank you to all of the new customers we have added in 2013. It’s been another growth oriented year for Intellicure, and we are excited about the possibilities of 2014. One of the changes I am implementing in 2014 is a month-to-month special for our existing customers on optional modules. Each month will feature a module or service offered by Intellicure which will be specially priced for that month only. We are kicking this off in January with one of our most popular options...Level 2 Reporting.

There are several advantages to Level 2 reporting. Level 1 reporting, which comes with the core system, provides the overview of activity in the software. However, Level 2 reporting provides the details down to the patient level allowing management to go the exact patient/location while knowing the staff and physician involved. Level 2 also combines clinic details allowing management to identify areas of concern or potentially missed opportunities.

Caroline Fife, MD, Honored at Recent Wound Care Symposium

Intellicure’s Chief Medical Officer, Caroline Fife, MD, was recently honored with two awards for her outstanding industry contributions at the Clinical Symposium on Advances in Skin & Wound Care (CSASWC).

The Eric P. Kindwall Award is presented annually to an outstanding American College of Hyperbaric Medicine (ACHM) member. This year, Dr. Fife was the recipient for her dedication to educating the healthcare community about the importance of hyperbaric medicine in wound healing.

Dr. Eric P. Kindwall is often referred to as the “Grandfather of Hyperbaric Medicine.” During his years of practice, Dr. Kindwall dedicated himself to research, publication, and education in the field of hyperbaric medicine. He is credited for writing the first textbook on hyperbaric medicine.

Also announced at the recent CSASWC was the Sharon Baranoski Founder’s Award which is given to honor the overall pursuit of excellence in the field of skin and wound care. Dr. Fife was nominated by her peers in the industry, and her contributions were honored with this year’s award.

The Sharon Baranoski Founder’s Award is given by Lippincott Williams & Wilkins, publisher of Advances in Skin & Wound Care and sponsor of CSASWC, in honor of Sharon Baranoski, MSN, RN, CWOCN, founder and symposium director. She is an educator and author of various nursing and wound care publications.

The Member's Portal: Are You Making the Most of Your Intellicure Tools?

Hello everyone out there in Intelliland! In this issue, I want to talk about one of the most under-utilized tools we offer our customers at Intellicure: The Member’s Portal. As I speak with our customers across the country, I am often presented problems that are easily solved with the use of the Member’s Portal. However, I often find customers are not aware that we offer this feature. So let me take a moment to introduce you to this useful tool.

To access the Member’s Portal, go to the Intellicure corporate website. On the top right hand side of the page is the word Member’s Portal, which when selected will allow you to enter your standard Intellicure username and password. Once the portal opens, you will want to click on the useful information link to open up all of the categories.

We offer a variety of training videos on a number of areas in the software that can be utilized for retraining or initial training for new staff. You can find a variety of forms, papers and information on important subjects such as coding changes, industry news, and policy changes as well as, meaningful use information to name a few. There is also a section of frequently asked questions with “How-To” instructions on a variety of topics.

The Member’s Portal is easy to navigate and provides a tremendous amount of information. This is just another way Intellicure is Harnessing Technology for Healthcare.

Attention: Big Changes Coming to Wound Centers in 2014

On July 8, the Centers for Medicare & Medicaid Services (CMS) released the outpatient prospective payment system (OPPS) proposed rule for 2014. This proposed rule has significant proposals which will shift OPPS away from the current “fee for service” system to policies that could, over time, support outpatient “bundled payments.” CMS believes these changes will incentivize hospitals to furnish services in the most efficient way, encouraging long-term cost containment. The proposed rule changes are numerous and it would take many pages just to summarize them.

The final rule will be released in November and will take effect Jan. 1. Comments were due to CMS by Sept. 6 and many wound care organizations submitted comments, particularly about the package pricing of skin substitutes which I am NOT going to discuss right now. I am going to mention just one thing which affects wound center reimbursement.

I am going to talk about the change to facility Evaluation and Management (E/M) billing. CMS has proposed collapsing several “level of service” codes into one new code, resulting in a single level of payment for each of the three types of visits. This is something that Intellicure knows a lot about.

Intellicure’s involvement in the development of wound center facility charge methodology dates back to 2005 when we met with CMS on this issue. In 2000, not long after OPPS was created, CMS instructed hospitals to develop internal hospital guidelines for reporting the appropriate visit level of service until Medicare could develop something. Then in 2005, CMS proposed a facility reimbursement system based on wound SIZE. While many organizations were upset about this proposal, only Intellicure had the data to demonstrate to CMS why it would not work. Intellicure presented data to CMS which showed that the wound sizes they selected to represent the various E/M codes would result in 90% of facility visits being billed at the lowest level of service (due to the relatively small size of most wounds). Intellicure presented to CMS the Acuity Scoring System which we had developed as a measure of hospital resource utilization, and CMS agreed that such a system would work better than wound size to calculate facility E/M. (We published the original article on Acuity Scoring in OWM in 2007 and re-published it on an even bigger sample OWM in 2012.)

So, it is fair to say that Intellicure “invented” the current method of wound center billing. Despite this, over the past 8 years, the majority of hospitals have not been able to properly implement any measure of “resource utilization” as method of wound center facility billing. Many hospitals continue to simply “match the level of service billed by the doctor,” or to use a system based on time, or even use no system at all!

CMS has never wanted a billing system which was valid ONLY for “wound centers.” They wanted a method that was consistent across all of OPPS. However, Medicare has concluded that adopting a set of national outpatient visit coding guidelines is just not feasible. It is unable to capture the wide variety of patient populations and service-mix provided across the country in all the types of clinics represented by OPPS, and has not been able to obtain consensus among all the stakeholders. CMS argues that its proposal to collapse the 20 visit codes into three HCPCS codes would reduce a hospitals’ administrative burden by obviating the need for the creation of systems such as the one Intellicure created for wound centers. CMS also believes that using larger payment bundles will incentivize hospitals to provide more efficient care. That may be true. So, what does this mean for wound centers?

The effect of this change is that all wound center E/M visits will be paid at the “middle” rate. We think this will actually be a good thing for wound centers. The U.S. Wound Registry contains data from over a million outpatient visits in which the level of service was calculated using the Intellicure Acuity Scoring system. Our assessment is that this change will not result in a negative financial impact for wound centers. Facilities which had failed to properly implement a system to measure resource use will be spared the risk of a RAC audit with potential repayment as a result. Facilities which HAVE been doing this right have been achieving a “normal” distribution of charges, which means that the majority of E/M charges were at the middle rate. Thus, there will be little change in payment, and perhaps a big decrease in administrative burden. So, despite the fact that Intellicure “invented” the wound center facility billing system as it now exists, we believe this change a good thing for wound centers.

Intellicure and Cytomedix Announce CMS Approval

Last month, Intellicure announced a new partnership with Cytomedix, a leading developer of patient-derived platelet healing technologies that harness the body’s natural healing process to improve and speed up recovery in wounds.

Cytomedix markets the AutoloGel™ System, a wound healing process that uses the patients own plasma to promote cell growth and form new tissue at a rapid rate.

Intellicure Announces Partnership with Cytomedix

Do The Right Thing Initiative™

The U.S. Wound Registry's Do the Right Thing™ initiative represents a model for the development and testing of the type of wound care quality measures which can be directly reported to CMS from a certified EHR. Naturally, there are many possible wound care quality measures. The Affordable Care Act mandates that starting in 2015, a substantial portion of a hospital’s revenue be linked to the reporting of quality measures. It is likely that in the near future, physician revenue will be similarly linked to quality metrics.

The wound care industry needs a collaborative approach to the selection, creation, validation and testing of an entire suite of quality measures (eventually electronically reported) so that we will not be left behind in the transition to a value based healthcare system.

The Health Information Technology for Economic and Clinical Health (HITECH) Act, enacted as part of the American Recovery and Reinvestment Act (ARRA), was signed into law in 2009, to promote the adoption and meaningful use of health information technology. Over the next 2 years, Intellicure spent almost $1 million dollars to change IntelliTrak into Intellicure Certified EHR. Intellicure in High Demand

As Intellicure continues to grow it is becoming more and more necessary to increase our capacity to serve clients. What you may not know about Intellicure is that although we are a small company we have the third highest usage of user sessions in Microsoft's central region. To meet this demand over the years we have added and replaced hardware for both physical and virtual environments.

Lately, however, we have accelerated this process to meet the needs of our new business lines for InRich Advisors, a subsidiary of Intellicure that provides consulting, and the U.S. Wound Registry, a non-profit organization dedicated towards data research. The addition of our ePrescribing service has also required updates and restructuring to our database server resources.

As some of you on the East Coast have experienced, there have been times this past summer where Intellicure had become slower than normal. We resolved that issue fairly quickly, however the next big leap will be near the end of the year, which is when we typically expect contracts to close and new clients to join.

What seems like a small list will be quite the task for our crew, but we have the experience and skills to meet these goals. We will send out more updates as we continue to progress. As always, thank you for being an Intellicure Client!

Intellicure Continues to Grow!

Intellicure has quadrupled in size since I first started, and we have shattered so many "glass ceilings" that we have begun to lose count of our major milestones. When I started Intellicure in 2007 my first office was a small desk shoved into the corner of an extremely loud server room, and we only had about 25 wound centers at the time. Once our Hosted system was officially launched in mid-2007 I spent the next few years flying by the seat of my pants in express jets and rental cars. I travelled to over 80 different cities across the US performing sales, project management, tech support and just about everything else that was needed of a young college graduate in a fast growing company. At some point in there we had only 5 employees and I was happily working as the grunt of the operations, I wish I could say "brains of the operation"; but I'm sure that's reserved for the actual smart people here at Intellicure. Fast forward to today and we're in a beautiful 5,500 sq. ft. office in one of the fastest growing cities in the country. We just hired our 19th employee, which means we are already bursting out of the seams and will likely have to move into yet another office soon.

It is hard to imagine all of this happened in only five years and under private ownership. I look around the office and it is very nice to see that all of our hard work has truly paid off. The astonishing part is that the party is just getting started. With the rollout of the Intellicure Certified EHR 5.1 we are pushing out new programs such as ePrescribing, Document Manager, Patient Portal, Reporting Services, extended DME services, and more. Our free time has become extremely limited. We are also in the process of expanding into yet another data center to fulfill our new resource needs. My role at Intellicure has never been more busy.


PHI: Keeping You Safe From the Risk of HIPAA Violations

What difference does it make if you have put PHI into one of your photographs? Many clinicians use photos containing PHI to present cases at meetings, and thus have these photos in their personal computer, or on a camera that might be stolen. If a clinician has photos containing PHI, even if you mask it for presentations, it still represents a potential HIPAA violation. Clinicians are urged never to store, maintain, display or otherwise utilize photographs containing PHI. However, as long as NO IDENTIFERS are put in the photograph, then there is no PHI at risk should your computer or camera be lost or stolen. We think that the best practice is simply NEVER to put PHI into the photograph in the first place.

We encourage our clients to use the Picture it There software available through Intellicure to drop photos directly into the EHR where they will be secure and encrypted. We discourage out clients from putting any PHI into the photographs. The substantial penalties for breaching healthcare privacy laws are avoided if the data are strongly encrypted. Since photos can't be encrypted while in a camera, the best place for them to reside is inside the EHR. And it is best to let the Intellicure EHR identify the photo, rather than putting PHI into the photograph.

Debridement Reports and Why Intellicure Created Them

Depending on how you calculate the effect of Obamacare, Medicare will go bankrupt in either 2024 or 2016, (2012 Annual Report of the Boards of Trustees of the Federal Hospital Insurance and Federal Supplementary Medical Insurance Trust Funds). Improper payments are a significant contributor to the financial instability of the Medicare trust fund. There are more than a dozen U.S. governmental auditing bodies currently at work attempting to recoup payments made to physicians and hospitals, and it is these repayments which are helping to keep Medicare afloat. Among the most active are the Recovery Audit Contractor (RAC) programs.

RACs engage in two types of claims reviews in order to identify improper payments: (1) automated reviews and (2) complex reviews. An automated review is a review of claims data without a review of the actual records supporting the claim. A complex review consists of a review of actual medical records and is used in situations where there is high probability that a claim includes an overpayment. RAC auditors work strictly on commission, receiving anywhere from 9%-12.5% of everything they collect, and they can extend their reviews as far back as 36 months. Many of these activities involve data-mining activities based on billing information.

The RACs use proprietary software programs to identify potential payment errors in such areas as duplicate payments, fiscal intermediaries' mistakes, medical necessity and coding. Remember that these auditors are looking for statistical anomalies. Thus, it is imperative that you know not only whether you can justify your billed level of service, but whether the distribution of your charges falls in an acceptable range from a statistical standpoint. This is why we have gone to the trouble to publish two papers in peer reviewed journals to prove that when used properly, the internal auditing features of Intellicure will produce a normal bell shaped distribution of facility charges. Intellicure is the only wound care EHR which internally audits the chart and which can demonstrate this capability.

Each RAC website lists the approved issues which they will target. Also, Medicare's Quarterly Provider Compliance Newsletter provides information on specific infractions, including problem descriptions and how providers can avoid them. The Department of Justice collaborates with many auditing agencies, including the Office of Inspector General. A RAC auditor may also target issues resulting from an OIG investigation. That leads me to why Intellicure created the Debridement report. Clinicians can be certain that debridement services are on the RAC work plan. You are well advised to understand what this report does, run it regularly and ensure that your debridement services are not only properly documented, but make sense in the context of the patient's overall care.

Document Manager: Intellicure's Newest Feature



Intellicure's new Document Manager feature will soon be replacing Patient Folder on your IntelliTrak launcher screen. Document Manager is a medical record system that will allow you to view read-only patient record documents with just a few simple clicks.

All of the patient information you need is displayed on the screen including demographics, known allergies, and documents generated for labs, studies and procedures. The Load Document feature will allow you to easily scan in outside documents needed for a complete patient history. Document Manager will work in conjunction with your facility's current document interface and will require no further action on your part.

Printing and Faxing Capabilities Within Document Manager



Two very important features within Document Manger are the Printing and Faxing Capabilities. Both of these features were created to save you time and improve patient care!

The Print feature will allow you to print a single document or a collection of documents. If an auditor needs access to all documents for a patient, one simple click gives you the ability to print an entire medical record at once. How simple is that?

Our Fax feature gives you the capability of sending documents to other facilities with a few simple clicks. Once you send a fax, you will be able to see the status and know for certain your document went to the intended receiver.

SAWC Spring 2012 - The Symposium on Advanced Wound Care

Stage 2 Meaningful Use: Are You Ready?

The Centers for Medicare & Medicaid Services (CMS) recent announcement of Stage 2 requirements for meaningful use of EHRs has caused quite a stir in the healthcare industry. The Notice of Proposed Rule Making (NPRM) for Stage 2 was published on March 7, 2012 with a 60-day comment period and final rule expected for the summer of 2012. One rule that stood out was the extension of Stage 2 attestation to 2014. Those attesting to meaningful use in 2011 will now have until 2014 to meet Stage 2 criteria and 2016 for Stage 3. Quality measures will be a distinct category in 2014. Eligible providers will now submit 12 measures and hospitals 24. Both must also select one measure from these categories: Patient Engagement, Patient Safety, Care Coordination, Population and Public Health, Efficient Use of Resources and Clinical Effectiveness. Another significant change is the proposal that Stage 1 optional (menu) items will become required (core) in Stage 2, although the concept of core and menu requirements will not change.

The requirement for Computerized Physician Order Entry (CPOE) for medications was changed from 30% in Stage 1 to 60% in Stage 2. The CPOE measurement was changed from being based on one order per patient to a percentage on all orders. These changes to CPOE will raise the bar considerably. With so many new changes, these are just a few examples of how Stage 2 is enabling the patient and putting them in charge!

By the end of January 2012, 65% of eligible hospitals and 36% of eligible providers had registered for meaningful use incentives. Stage 2 will be here soon and waiting until the final rule to get the ball rolling is not a good option. Now is the time for your organization to work toward meeting these requirements and earn your incentives.

IntelliCamp - New Client Managers Orientation

IntelliTrak 4.0 Upgrades are Currently Being Installed

IntelliTrak 4.0 is a part of your standard yearly upgrade offered by Intellicure. For most clients, no action is required on your part, this update is implemented internal to Intellicure and done by our support team. No changes will be made to HL7 interfacing with your hospital, however, if your HL7 Interface Team wishes to perform HL7 testing with the new version, we are more than happy to work with them. The 2012 CPT and ICD-9 updates have already been applied to your database. However, if you have not provided us with your 2012 Skin Substitute Chargemaster Codes, the existing Skin Procedure Codes will be deactivated. Release Notes, new Training Manuals and Training Videos will be provided and accessible in the Member's Portal. Certified Intellicure EHR, Version 5.01 Now Available!

What does certification mean? Having a certified EHR means that physicians and facilities can meet the federal guidelines for "Meaningful Use of a Certified EHR," and thus qualify for bonus money (up to $44,000 per doctor)!

Nearly 24 months of work and almost $1 million went into bringing our certified EHR to market. We are proud of the advances we have made and know you will be impressed. However, the additional work related to the certification, and the licensing we pay for certain features, such as electronic prescribing (eRx), means there is a need to increase the monthly charges. To say "Thank You" to all of our existing clients, we have decided to offer "early adopter" pricing to current customers who sign up for the certified version. We also realize that some clinicians will achieve "meaningful use" at other practice locations, and that some clinics will prefer to simply upgrade their current software. Intellicure Upgrades - A Letter from Dr. Caroline Fife



I feel like a kid in a candy store. I just got access to IntelliTrak 4.0 in my clinic. I am like the cobbler's children who have no shoes. The complexities of the Medical center always make me the last person to get the latest upgrades!

What I really want is the certified product (5.1), but since the Medical Center has chosen the non-hosted version, the certified product is not an option right now. Still, life for me just got a whole lot better. Different people get excited about different software features. The one I am most excited about getting is OrderTrak. I can't tell you how much I hate filling out DME supply orders. Somehow I get stuck with a substantial portion of that paperwork, and I have always hated it. OrderTrak automates this process and I can't wait. The possibility of just cutting in HALF the stack of papers on my desk waiting for a signature is exciting to me. And that brings up the issue of the document manager and how we are going to manage outside records. No more running around with those thick charts. We really can go electronic now. I may be the only one who knows what has happened on the back end to improve security with regard to encryption and authentication, but now we have what we need to meet the current standards. Happy sigh.

I am missing some things I REALLY want however, because I don't have the certified product. What are they? There are some big differences between 4.0, even with ePrescribing, and 5.1. The biggest ones revolve around the automation of the quality measures and the ability to submit your data so you are eligible for your HIT adoption bonus money. However, the patient portal feature and the educational resources that are available with 5.1 (including the ones in Spanish) would be a huge help to me and I hate not having access to that.

There is a lot to learn about these different products. If you use our certified product (5.1) you can quality for bonus money under stage I of meaningful use. If you use 4.0 or 4.0 with eprescribing, you won't get all the newest features and you can't get your bonus money, but you will still get a lot of great advancements which are worth having.


Dramatic Reimbursement Changes Go Into Effect January 1, 2012

For CY 2012, the AMA's CPT Editorial Panel has once again radically changed the billing guidelines for semi synthetic skin substitutes. This time, they have deleted the current 24 skin replacement and skin substitute related CPT codes, and replaced them with 8 completely new CPT codes in an attempt to describe more accurately the services associated with skin replacement procedures. 24 CPT codes in the range between 15170 through 15431 have been deleted. 8 CPT codes in the range between 15271 through 15278 are new and will be effective January 1, 2012.

The biggest change that wound care clinics will see are the code pairs currently used for application of Apligraf, Dermagraft, and Oasis. The new codes are grouped by anatomical location and then by wound size range into primary and add-on code pairs. Thus, replacing current code pairs and adding a second code pair to each of the applications.

Contact your Chargemaster Coordinator to make sure the new codes have been added to your department's hospital chargemaster so that you remain compliant. Failure to act could cost the average wound care clinic over $10,000 per month.
