DFU/VLU Skin Substitute LCDs and Billing Articles by MAC

Below is an updated, consolidated reference list of Medicare Administrative Contractors (MACs) with their corresponding Local Coverage Determinations (LCDs) and Billing & Coding Articles governing the use of skin substitute grafts / cellular-and-tissue-based products (CTPs) for diabetic foot ulcers (DFUs) and venous leg ulcers (VLUs). All LCDs listed below are future-effective January 1, 2026. We provide this list to you as a courtesy as part of our December 2025 eBook on the 2026 Skin Sub policy changes.
